New Patient Registration
You are very welcome to come in to reception to register, but if you would prefer to save time by doing the paperwork before you come everything you need can be found on the above "Registering Online" tab. Please complete one copy of each document for each person registering. If you are registering in person, please bring ID with you when you return your completed forms.
We are currently accepting new patients from:
- Cranleigh and Ewhurst
- Alfold Bars, Alfold, Dunsfold
- Bucks Green, Rudgwick
- Ellens Green, Cox Green
- Walliswood, Forest Green
- Rowly, Shamley Green

At Cranleigh Medical Practice you will be given a named GP who will have overall responsibility for your care. As a practice we offer you the option of seeing a GP of your choice if your GP is not available or if you prefer to see someone else. Please ask if you are unsure who your named GP is.
Please read the information here about Summary Care Records. If you do NOT want your records uploaded you will need to print out and sign this opt-out form.
When you have registered, you will be offered an appointment with one of our healthcare assistants for a New Patient check-up - we recommend you take up the offer because it's a great way to find out about the services and clinics we offer here, as well as getting an up to date blood pressure and urine check. If you have an on-going health problem, such as diabetes, blood pressure or asthma, our healthcare assistant will make sure you are put on recall for the appropriate specialist nurse led clinic.
