New Patient Registration
You can register online by completing the Registration Form or you can register in person at the Practice. To check if you are in our Practice Area click here.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
Once you have completed the Registration Form, please complete the Online Medical Questionnaire For New Patients. This is to give us information we need about your health before your notes arrive from your previous practice.
If you are a Carer please use the following form to register with the practice as a carer Carer Registration Form
At Cranleigh Medical Practice you will be given a named GP who will have overall responsibility for your care. As a practice we offer you the option of seeing a GP of your choice if your GP is not available or if you prefer to see someone else. Please ask if you are unsure who your named GP is.
Please read the information here about Summary Care Records. If you do NOT want your records uploaded you will need to print out and sign this opt-out form.
When you have registered, you will be offered an appointment with one of our healthcare assistants for a New Patient check-up - we recommend you take up the offer because it's a great way to find out about the services and clinics we offer here, as well as getting an up to date blood pressure and urine check. If you have an on-going health problem, such as diabetes, blood pressure or asthma, our healthcare assistant will make sure you are put on recall for the appropriate specialist nurse led clinic.
